Contact the administrator for your 401(k) - or the HR from your old job - and request a rollover form and whatever information you need to provide to them to roll your money to USAA.
When I moved from CA to PA, all I had to do was go to the local DMV office with my old DL, and proof of residency, and they happily gave me a PA license. No test or anything else needed. I was even able to register to vote at the same time.
